What causes type 2 diabetes mellitus ?

Type 2 diabetes mellitus is a multifactorial disease ,which means it has many causes, but all these factors ultimately lead to carbohydrate intolerance and insulin resistance in the patient leading to development of symptoms.

Diabetes reversal as defined by American diabetes association (ADA) and European association for study of diabetes (EASD) refers to a metabolic state in which a person previously diagnosed with type 2 diabetes achieves and maintains normal blood glucose levels without the use of glucose-lowering medications for more than three months.

“Diabetes reversal” in medical terminology is also called "diabetes remission".

In this context, "normal blood glucose levels" typically refer to:

  • Fasting plasma glucose (FPG) < 100 mg/dL
  • Hemoglobin A1c < 6.5%
    without the use of any antidiabetic drugs.

Current Conventional diabetes management has historically prioritized and focussed on glycemic control—that is, maintaining blood glucose within target ranges to reduce the risk of complications. But these approaches do not act on the underlying pathology , hence the current conventional treatment with medicines does not stop the progression of the disease and the disease continues to progress and gets worse and worse with time in spite of treatment, even if the blood glucose is within normal range with treatment.

Emerging evidence shows that simply lowering blood glucose with medications does not adequately address the metabolic dysfunctions underlying type 2 diabetes, nor does it fully prevent the long-term complications associated with the disease. Current conventional treatment with medicine can only slow down the onset of complications but can not prevent it.

    Most glucose-lowering medications function by enhancing insulin secretion, few medicines act by increasing insulin sensitivity, or reducing glucose absorption. While these actions lower HbA1c and fasting glucose, they do not eliminate insulin resistance or reduce visceral and ectopic fat ( hepatic, pancreatic, cardiac, intramuscular fat, etc). In fact, some medications—such as insulin and sulfonylureas promote weight gain, increase insulin resistance and worsen the disease over time.

    Studies like UKPDS and DiRECT have shown that early and aggressive intervention—especially when focused on weight loss—can lead to sustained metabolic benefits, even if glycemic control deteriorates later. This “legacy effect” supports the notion that reversal, when achieved early, may reduce long-term cardiovascular and renal complications.
  3. Medication Burden and Adverse Effects
    Prolonged pharmacotherapy carries risks, including hypoglycemia ( occurs frequently with sulfonylureas and insulin), gastrointestinal side effects, and long-term progressive disease burden. Some agents (e.g., thiazolidinediones) have been associated with heart failure or fracture risk. Diabetes reversal allows for deprescribing or minimization of drug dependence. Sulfonylureas, insulin and few other drugs may be beneficial in the short term but in fact cause more harm than good in the long run.

What is low carbohydrate diet ?

Low carbohydrate diet in simple terms means eating less carbohydrate in the food.

Many diet patterns have been claimed to reverse type 2 diabetes  (e.g. , low carbohydrate diet, very low calorie diet, plant based vegan diet, Atkin’s diet, carnivore diet, etc ), but out of all these, the low carb diet has the most scientific evidence and is the most researched dietary treatment for diabetes reversal. American diabetes association (ADA) has in its standard of care  stated that low carb diet has the most scientific evidence for maintaining euglycemia. Low carb diet has the highest rate of type 2 diabetes reversal as well as long term sustainability.

How low is low carb diet ?

The exact definition of what is a low carb diet is not universally established.  Different organisations have different opinion about it.

 In general, any diet in which carbohydrate is less than 130 gm may be considered low carb diet. To be therapeutically useful in reversing the disease, carb consumption should generally be less than 100 gm per day.

Some patients get benefit with only little carb restriction whereas some require more carb restriction. It needs to be individualised depending on the need.

How much carb a type 2 diabetes patient may eat depends upon the stage of the disease and patient’s metabolic status . It varies from person to person. It needs to be individualised.

 What It Involves :  usually less than 100g carbs/day, often <50g for more rapid reversal, depending upon the patient need.

Emphasis is on:

Increased intake of Non-starchy vegetables and green non starchy salad.

Reduction of high carb food ( e.g cereals( rice, wheat), millet, pulses, legumes, etc)

Note :

although cereals and millet both contain about 80% carbohydrate but  millets are much better than cereals because their carbohydrate is slowly absorbed leading to less glucose spike. some patients may tolerate some amount of millet and pulses and even cereals, depending upon their disease stage. Pulses (daal) and legumes contain about 30 – 50 % carb and hence they are better than cereals and millet. How much a patient can tolerate is variable.

Increased intake of low carb natural / minimally physically processed ( no chemical processed food) food items e.g paneer, cheese, tofu, mushroom, eggs, green vegetables, green salad,  and all non veg food items .

Increased intake of Healthy fats (olive oil, ghee, butter, coconut oil, small amount of cold pressed mustard oil and cold pressed flax seed oil, natural fat present inside nuts and seeds, and all naturally occurring fat present in animal or plant food).

Refined vegetable seed oils must be avoided because they are highly inflammatory , obeisogenic and diabetogenic, increase risk of coronary artery disease (CAD). These harmful effects are slow process , hence people usually do not realise their harm on short term use and later blame on other factors for the harm caused by refined seed oils and high carbohydrate foods ! ( people also get mislead by the misinformation campaign by various organisation and by misleading advertisement on TV channels by artificial ultra processed food manufacturing industry).

Pl note that refined oils ( vegetable seed oils) which contain very high amount of omega 6 fatty acids are very harmful oil as per credible scientific studies ( e.g. Minnesota heart experiment, Sydney diet heart study, Rose corn oil study, etc) and must be avoided to minimum possible or stopped altogether.

pl note that promotion of refined vegetable seed oil by various organisations is due to their commercial and financial interests by ignoring the credible scientific evidence against these refined oils (the evidence resistance) and also because of conflict of interest in the recommending organisations under influence of artificial food manufacturing industry.

Intake of Quality protein in recommended normal amounts required by the body (eggs, meat (the meat should not be factory processed or preserved meat), fish, tofu, paneer, minimally processed cheese, etc) . Eating adequate amount of protein in the diet is very important for the long term success and sustenance of low carb diet.

(pl note that low carb diet is not a high protein diet by default !)

Minimal or no refined carbs ( e.g. wheat, white flour, factory produced slice bread, sweets, pastries, etc)

Minimising artificial highly processed factory produced junk food items available in packed form .

Is this treatment safe ?

This Low carb treatment for diabetes reversal is a well researched and scientifically proven to reverse diabetes in majority of eligible patients, in a safe and effective way. There are plenty of credible scientific research evidence to support this.

The American diabetes association (ADA) says that the low carb dietary intervention has the most scientific evidence for treatment and reversal of type 2 diabetes.

Low carb dietary treatment for type 2 diabetes beneficial  not only in type 2 diabetes but also improves mental alertness, reduces requirement of blood pressure medicines, reduces obesity, improves metabolic health, reduces inflammation inside the body, improves many autoimmune diseases severity, reduces heart attack risk by reducing triglycerides and increasing HDL, etc. However, there are some issues with 20 % patients in which it may increase cholesterol. However, in research studies it has been seen that this diet increases good cholesterol and reduces bad cholesterol and also reduces Triglycerides and increases HDL leading to overall heart risk reduction in the patients. Also, the increased cholesterol in these patients comes down to original level if the patient starts eating high carb diet again ! 

Low carbohydrate dietary treatment with higher amount of healthy natural fat is a safe, effective and scientifically proven treatment for diabetes reversal as per most of the credible scientific research and scientific evidence. Any controversy which is created is only by those people who lack the latest scientific research knowledge and who are not able to interpret the scientific data appropriately.

My diabetes is 20 years old. Can I reverse it ?

YES. Although the chances of reversal are more if the diabetes is of shorter duration, but the type 2 diabetes can be reversed even after many many years of existence. So, every patient should try to reverse diabetes whatever may be the duration of his/her disease. chances of complete reversal gradually decrease with longer duration of the disease but it is possible.

Side Effects of Low-Carb Diets and How to Prevent Them

While low-carbohydrate diets are safe and effective for many people, especially those with type 2 diabetes, the diet transition can sometimes cause temporary side effects because our body needs time to adapt to any type of change. Most of these are easy to handle, manageable, preventable, and not harmful in the long run. These side effects occur in minority of people only. This chapter addresses the most common issues, their underlying causes, and evidence-based strategies to avoid or manage them.

Most of these side effects usually do not occur if the transition to low carb diet is done slowly and gradually. In my practice , none of my patients have observed these side effects due to gradual diet transition recommended by me.

  1. Keto Flu (Low-Carb Adaptation Symptoms)

What it is:
During the first few days of carbohydrate reduction, some people experience flu-like symptoms if the shift to low carb diet is rapid : e.g. Headache, Fatigue, Dizziness, Nausea, Muscle cramps, Brain fog. All these symptoms go away once the body adapts to the new diet within a week or two.

Why it happens:
As your body shifts from burning glucose to burning fat (and ketones), it also sheds water and electrolytes like sodium, potassium, and magnesium. This may lead to dehydration and electrolyte imbalance.

How to prevent/manage it:

  • Stay well-hydrated (2.5–3.5 liters/day) , Add adequate salt and electrolytes, magnesium , etc. , Rest during the first few days , Reduce exercise intensity temporarily
  1. Constipation

Why it happens:
Reduced intake of fiber-rich carbs and dehydration in a poorly designed low carb diet can affect bowel movements.

Solutions:

  • Drink more water , Eat low-carb, high-fiber vegetables (spinach, cabbage, cucumber) , Add 1–2 tablespoons of chia seeds or flaxseeds or isabgol husk , Ensure adequate healthy fat intake (which helps stool lubrication)
  1. Nausea or Digestive Upset

Why it happens :
Rapid increase in dietary fats — especially if the body is not used to digesting them.

Solution:

  • Gradually increase fat intake over 1–2 weeks , Choose healthy fats: ghee, butter, olive oil, coconut oil, nuts, seeds, animal fat , Avoid deep-fried or greasy foods , Eat smaller, more frequent meals temporarily for some time.

 

 

  1. Frequent Urination and Increased Thirst

Why it happens:
Glycogen (stored glucose) is released and burned, and every gram of glycogen holds water. As this is used up, water and sodium are excreted, increasing urination.

Tip:

  • Increase water, fluid and salt and electrolyte intake early in the transition , Avoid sugary drinks, use lemon water, coconut water, or bone broth (unsweetened)

 

  1. Fatigue or Low Energy in Early Stages

Common in first 3–5 days if diet transition is rapid

Why it happens : Body has not yet adapted to using fat and ketones as fuel.

Prevention:

  • Eat enough calories (especially from fats and proteins) , Don’t restrict carbs too drastically at first — gradual reduction helps , Support with adequate sleep and light activity.
  1. Hypoglycemia (Low Blood Sugar)

Low carb diet never causes hypoglycemia. Diabetic medicines cause it.

Who is at risk : People on insulin, sulfonylureas, or other glucose-lowering medications may develop hypoglycemia if medicine dose is not reduced before changing to low carb diet.

Why it happens : Blood glucose starts to drop with carb restriction, but if medications are not reduced, glucose can go too low due to effect of glucose lowering medicines.

Signs of hypoglycemia: Sweating, Dizziness, Palpitations, Confusion, Shakiness, slurring of speech

Prevention:

  • Work with your doctor to reduce medications before the diet transition. Monitor glucose frequently. Always carry glucose tablets or a small carb source when adjusting medications and use it when necessary.
  1. Muscle Cramps

Why it happens :
Loss of sodium and magnesium can lead to cramps, especially at night or after exercise.

How to manage :

  • Sodium: Add pinch of pink salt/ Himalyan salt or drink homemade bone broth
  • Magnesium: elemental 300–400 mg/day , 1000 mg of magnesium salt (consult doctor before supplementing)
  • Stay hydrated and stretch regularly

 

  1. Bad Breath (Ketone Breath)

Cause:
Increased production of acetone (a ketone) exhaled through the lungs. It may occur in people eating very low carb ketogenic diet. It does not occur if the diet contains more than 50 gm carb per day and if the body is not in nutritional ketosis.

Management: Improve hydration, Use sugar-free mouthwash, Often fades as the body adapts to ketosis

 

  1. Sleep Disturbances

Why it happens :
Some people experience changes in cortisol and other hormones as the body adjusts to new energy metabolism. It is temporary and Sleep becomes better once the body adapts to new diet.

Management : Avoid caffeine late in the day , may eat a small protein containing snack before bed for a few days initially , Magnesium supplementation may help sleep quality.

  1. Nutrient Deficiencies (if diet is poorly planned)

Does not occur in well formulated low carb diet because well formulated low carb diet is nutrient dense diet. Risk arises if diet is poorly formulated or overly restrictive.

Management : Ensure intake of omnivore natural whole foods and vegetables

  • Magnesium: nuts, seeds, spinach
  • Potassium: avocado, leafy greens
  • Vitamin B-complex: eggs, meat, leafy vegetables
  • Calcium: curd, paneer, nuts
  • Protein : eggs, meat, fish, paneer, cheese, tofu, soya chunks, curd,
  • Fat : butter, ghee, extra virgin olive oil, cold pressed coconut oil, cold pressed mustard oil,

A well-formulated low-carb diet includes lots of vegetables, nuts, seeds, dairy, meat, fish and healthy oils, which together provide most required micronutrients for the humans.

However, studies show that no nutrient deficiency occurs even on completely carnivore diets. Have you seen any adult lion or tiger who eat only meat, nutritionally deprived ? It is the plant based vegan diet which causes nutritional deficiency, not a well formulated low carb nutritionally dense diet.
